Respected Sir,

I Shishutosh Paul here is inform you that I am a Voter of 54 West Guwahati Constitutions, Polling Station No 237 (New). I am casting my Vote from 1993 at Odalbakra L.P. School at Odalbakra & my Voter Sl. No. was 5. But in the mean time my name was transferred to Samabay Prabandak Sangstha at Kahilipara. I casted my vote there also for MP Election on 2001 & there is my Voter Sl. No.3. I cast my vote at GMC election on 2004 at Modern English School at Kahilipara & Voter Sl.No.3. But when the Sangstha was transferred from the place at Kahilipara to Dispur & that Poling Station further transferred to Odalbakra L.P. School at Odalbakra. But unfortunately my name was omitted from the Voter List from 2005. From that time I am applying for my voter rights & my name to be listed in voter list. Now I am failure on submitting Form No.6 after so many times submitted.

In 1995, when the Government tries to issue Voter I.D., your person was taken my snap & given me Identity Slip also. That was 54 West Guwahati LAC, Part No.105 SL No. in Electoral Roll 780 & Photography SL No.A-80.

Sir, now I wise to know who is the person, who omitted my name from Voter List & why? As a general public where I go for Voter Rights, please help me & advised. If you say for Appeal, I agree. If you wise to see the documents, I must be produced & all the Voter List of previous year lying with you, please see & take necessary action.
